If you haven’t submitted your second half 2008 semi annual reports to Ruth at the Highland County Chamber (fax (937) 393-9604) your report is late. We need these reports to close out the calendar year 2008 and record your business/organization’s data for the frequency and severity performance portion of the safety council discount incentive.

If you have questions or need help with this form, please call Ruth (393-1111) - she’ll be happy to help you with the details.
